Lost or stolen phone?

Please call the cellular carrier to report the phone lost or stolen. They will suspend the service. You must also report this to UITS Cellular services (860)486-4357 as soon as possible so a replacement device may be ordered. You cannot order a phone directly from the carrier.

What do I do with an old cell phone?

  1. If the cell phone still has service on it, submit an online Disconnect Request.
  2. You can choose to:
    • Keep it to use for another employee or have as a spare in case your current device breaks.
    • Complete an ACT-39 form and send to Central Stores. They will recycle them properly. Be sure all information has been removed.
    • Return to UITS-Cellular Services. Please label the device “good” or “bad”.

PLEASE NOTE:  Returning cellular equipment with service on it will not be considered a request
to disconnect. A
Disconnect Request must be submitted to stop billing for services.

Be sure to call from a different phone and have your mobile device ready to troubleshoot. We are not responsible for setting up personal phones, but we will do our best to assist you.

What are my options regarding Used Equipment?

  • If the cell phone still has service on it, submit an online Disconnect Request.
  • If the equipment is in GOOD working condition, the department can reissue to another employee needing cellular service to perform their work duties.
  • Keep the device as a spare in case your current device stops working.
  • You may return any GOOD equipment to UITS/Cellular Services, Unit-3138.
  • BAD equipment (or really old equipment) should be recycled at Central Warehouse accompanied by a completed ACT-39 form.

Who is eligible to participate in the Wireless Stipend Program?

Only “Management Exempt” employees are eligible to participate in the Wireless Stipend Program.
